• Copays and coinsurance for doctor visits are required at the time of your appointment with no exceptions.
  

• Coinsurance and deductibles are required when surgery is scheduled.
  

• We accept:  Mastercard, Visa, Discover, debit cards, cash, and certified checks.
  

• Sorry, but we do not accept personal checks.
  

• Payment plans are available for self-pay patients.  If you are not filing insurance or have no insurance, $150.00 minimum is due at the initial visit.
